It is our pleasure, at Wellness From the Inside Out, to welcome you as a new chiropractic patient. For your first visit, please plan on arriving 10 to 15 minutes early to our Lake Zurich chiropractic office to complete some brief, but necessary paperwork. This will help us to get to know you and have a better understanding of you and your condition. Your initial visit will take approximately 30 minutes.
If you have insurance, please bring your card and/or information with you. In an effort to maximize efficiency, you can download and fill out the necessary paperwork. Once you have completed all of the necessary forms, you can then fax it in to the office ahead of your initial visit. We will have your information ready for you upon your arrival to review it with you in case there are any questions. Our fax number is (847) 550-1117.

Once we receive and review your paperwork, you will be given a consultation with one of our highly qualified chiropractors. They will discuss with you your health-related problems, concerns and chiropractic treatment options.
Our first concern is to determine your actual condition. The doctor will ask a series of questions related to your condition and perform a number of specialized tests in order to determine which areas are affected and to what extent, providing you with a diagnosis.
After the doctor has completed your history and examination, it is possible your specific condition may require x-rays or other imaging tests. This is done to help diagnose or rule out more serious conditions as well as to assist us in developing the most effective treatment plan for you. If during your visit x-rays are required, we will refer you to one of the many nearby and convenient locations.
Once we have completed your initial examination and any appropriate studies, you will most likely receive your first chiropractic treatment during this same office visit. This may include spinal adjustments, physical and/or soft tissue therapies or acupuncture, depending on your level of pain.
Prior to leaving our Lake Zurich chiropractic office, we will provide you with instructions as to what certain activities and/or procudures we recommend for you to complete at home. This may include ice or heat application instructions; avoidance of certain activities or positions; and home exercises and/or stretches.
Finally, we will schedule an appointment date and time for your follow-up visit that is convenient for you. You should generally expect to be seen at our Lake Zurich office within 1-2 days after your initial visit for a complete review of our findings. At this time, one of our doctors will discuss treatment options along with a treatment plan for your specific condition(s). We will also go over your current insurance coverage and any other obligations at that time.
